Individuals, families, representatives, and Supports Coordinators may contact us. Services must be authorized through the individual’s ISP before delivery begins.

Which waivers do you serve?

Dakota Connections serves individuals through the Consolidated, P/FDS, and Community Living waivers, based on service authorization.

Where are services available?

We serve Philadelphia, Montgomery, Bucks, Berks, Chester, and Delaware counties.

How does a referral begin?

Complete our basic referral inquiry or contact us by phone or email. We will discuss the requested service, county, authorization, and next steps.

Do you provide nursing care?

Our listed ODP services are non-nursing supports. Dakota Connections does not provide skilled nursing through these services.
